The following information must be on the note:

EVERY DAY (PERMANENT) PICK-UP:

• The note must indicate who will be picking your child up. Please list all names of people who may be picking up throughout the school year.
• One note is sufficient for the whole year.
• Indicate that it will be a permanent pick-up.
• Your child’s first and last name.
• Your child’s I.D. number.
• Your Signature.

OCCASIONAL PICK-UP:

• A note is needed every time you pick up.
• The note must indicate that pick-up will be for that day, with the date.
• Who will be picking your child up.
• Your child’s first and last name.
• Your child’s I.D. number.
• Your Signature.
